Our first instinct is always to save a tooth. Extraction is what we recommend when saving it is no longer realistic, when decay has destroyed too much structure, when a fracture runs below the gum, when gum disease has taken the supporting bone, or when a wisdom tooth is impacted and causing repeated infection.
A tooth that is beyond saving is not neutral. It is a source of infection that can spread to the bone and neighbouring teeth. Removing it cleanly, and planning what replaces it, is often the treatment that protects the rest of your mouth.
Signs you may need this
- Severe pain from a tooth broken below the gum line
- Repeated swelling or infection around a wisdom tooth
- A tooth that has become mobile due to advanced gum disease
- Decay so extensive that no filling or crown can restore the tooth
What happens during your appointment
Imaging and assessment
A digital X-ray shows the root shape and its relationship to the nerve and sinus, essential for planning a clean, safe removal.
Complete numbing
Local anaesthetic is placed and we confirm the area is fully numb before starting. You should feel pressure, never sharpness.
Removal
The tooth is loosened from the ligament that holds it and lifted out. Impacted or curved-root teeth may be sectioned into pieces so they come out through a smaller opening. This is gentler on the bone, not more aggressive.
Socket care
The socket is cleaned, and a stitch is placed if needed. You bite on gauze for 30 to 45 minutes to form a stable clot.
Planning the replacement
Before you leave, we discuss what fills the gap, whether an implant, bridge or denture, and the ideal timing for it.
Why it is worth doing
- Removes a source of infection before it spreads to bone
- Relieves pain that no painkiller was reaching
- Prevents damage to neighbouring teeth from impacted wisdom teeth
- Creates space where teeth are crowded, if orthodontics is planned
- Digital X-ray planning means a more predictable, gentler removal
Aftercare
- Bite firmly on the gauze for 30 to 45 minutes without checking it repeatedly
- Do not rinse, spit forcefully or use a straw for 24 hours. This dislodges the clot
- No smoking for at least 72 hours; it is the single biggest cause of dry socket
- Cold, soft food for the first day. Chew on the other side
- From day two, rinse gently with warm salt water after meals
- Take prescribed medication as directed and complete any antibiotic course
- Call us if pain worsens after day three rather than improving
Teeth Extractions: common questions
The questions patients actually ask about this treatment, answered without the parts that only apply to someone else's case.
The extraction itself should not hurt. The area is fully numb and you feel firm pressure rather than pain. Afterwards, expect soreness for two to three days, which is normally well controlled with the painkillers we prescribe.
The gum closes over within 1 to 2 weeks. The bone underneath continues remodelling for around 3 months, which is why implant timing is planned carefully.
Dry socket happens when the blood clot protecting the socket is lost too early, exposing bone. It causes severe throbbing pain around day 3. Avoid it by not smoking, not rinsing forcefully, and not using straws for the first 24 to 72 hours. If it happens, come in. We can dress the socket and the relief is immediate.
For back teeth, usually yes. When a tooth is removed, the teeth beside it drift and the opposing tooth over-erupts, which changes your bite and makes later treatment harder. Wisdom teeth are the exception. They generally do not need replacing.
